Pet Health Certificates

To schedule a health certificate appointment for your pet, please follow these steps:

Initial Contact: 

• Call us and we’ll provide you with the Visit Preparation Worksheet.
• Complete the worksheet and submit it for review before scheduling an appointment.
• The process takes a minimum of 2 weeks (longer for Hawaii/Australia), and same-day requests or walk-ins are not accepted.

Review Process:

• After we receive your completed worksheet, a doctor will review it and determine when the appointment can be scheduled.

Appointment Scheduling:

• Once approved, your appointment will be scheduled for 1 hour to allow time for the exam and certificate completion.

Post-Appointment:

• After your appointment, ensure you leave with all necessary documentation. If anything is missing, we’ll work with you to resolve it.

Key Notes:

• The Visit Preparation Worksheet is required before scheduling.
• Ensure you include specific country requirements on the form.
• The process takes at least 2 weeks (longer for Hawaii/Australia).
